For new patients to our practice we have provided the New Patient Registration Packet below.  You can print this document and complete it prior to your visit to provide us with standard registration information, some medical history and to indicate your understanding of our HIPAA compliance responsibilities.

The Registration/History Update Form is for our returning patients.  If you are a patient of SSGA, but haven't been seen in a year or more you should print and complete the Registration/History Update form to provide us with the most up to date information.
 
The documents listed below are used to collect the information we need to provide medical services to our patients, and must be completed prior to your appointment.  Completing the appropriate document listed below, prior to your visit, will help us to more efficiently provide you with the high quality of medical service that you can expect from Saratoga Schenectady Gastro Associates and save you the trouble of having to complete them in the waiting room prior to your appointment.
