VACCINATION CARD

As of May 19th, new york state is not requiring covid testing prior to elective procedures if patient is fully vaccinated.

Fully vaccinated means:

Patient has received two doses of Moderna or Pfizer and two weeks have passed since the second vaccination date
OR
The patient has received one dose of Johnson & Johnson and two weeks have passed since the vaccination date.
